About Our Fees

The Sheboygan Surgery Center has established fees for each procedure and we adhere to that fee regardless of minor variations in supplies. Our Surgery Center fees include items such as the cost of the procedure, operating room, supplies, equipment, and recovery care. On occasion, some managed care providers do not cover the cost of implants such as ophthalmic lenses, plates, screws and other implants used during the procedure. In that case you may be asked to reimburse us for those items. Our fees do not include charges from your treating physician. Depending on additional services required during your stay, you may also receive separate bills for lab work, anesthesiology, imaging services, pathology and radiology.

Out of Network Coverage
If the Sheboygan Surgery Center is considered an out of network provider or is not listed as a “participating provider” with your insurance company, we offer an in-network coverage program so you will not pay anything extra to obtain your health care from the Sheboygan Surgery Center. This means you may use our services and pay the same deductible and co-insurance you would pay if we were an in network provider facility—additionally, we will waive the additional out-of-pocket payment your benefit plan requires for use of an out of network provider.

How Your Bill is Processed
Our scheduler will contact you prior to your admission to review your insurance coverage and projected cost of services. Please note, sometimes your plan of care changes from the time of admission to the time of discharge, so there might be charges added to your bill after you are discharged. Co-pays, deductibles and co-insurance are due at time of service. If you are unable to meet this requirement we do offer an interest free short term payment plan.

Financial Assistance Programs
The Sheboygan Surgery Center does not penalize or deny anyone service if they have an inability to pay. To receive financial assistance you may be asked to provide financial information regarding your income, financial assets and liabilities. We also use The Department of Health and Human Services Federal Poverty Guidelines to determine a person's eligibility for financial assistance. If you feel you may be in need of assistance, please contact our billing representative at (920)453-7305.

Please be sure to bring your insurance card and photo ID the day of your scheduled procedure.
